This notice describes how your medical information may be used and disclosed, and how you can get access to this information. Please review it carefully.

How We May Use Your Health Information
Treatment To provide and coordinate your healthcare services. Outside referrals require your written authorization.
Payment To bill and collect payment. Out-of-pocket payments may be kept from your insurer upon request.
Operations Internal quality review, staff training, accreditation, auditing, and business management.
Public Health As required by law — such as reporting communicable diseases to public health authorities.
Legal Requirements When compelled by court order, law enforcement, or government oversight activities.
Communications To inform you about services, educational programs, or health-related activities we think may interest you.
Special Protections for Sensitive Information
Psychotherapy Notes Your written authorization is required for most uses and disclosures of psychotherapy notes — beyond standard HIPAA protections.
Substance Use Disorder (SUD) Records — 42 C.F.R. Part 2 SUD treatment records carry additional confidentiality protections under federal law. These records generally cannot be used or disclosed without your specific written consent, and may not be used in criminal investigations without a court order. You may revoke your consent at any time.
HIV-Related Information Most disclosures of HIV-related information require a HIPAA-Compliant Authorization for Release of Confidential HIV-Related Information. Certain public health reporting is permitted by law without consent.
Your Rights Regarding Your Health Information
  • Right to Access & Copy — Request a copy of your health records in writing. Allow approximately 2 weeks. A copying fee may apply.
  • Right to Amend — Request corrections to information you believe is inaccurate or incomplete. Submit in writing with your reasons. We will respond within 60 days.
  • Right to Request Restrictions — Ask us to limit how we use or share your health information. We are not required to agree to all requests, except where required by law.
  • Right to Confidential Communications — Request that we contact you at an alternative address or by alternative means.
  • Right to an Accounting — Receive a list of certain disclosures we have made outside of treatment, payment, and operations.
  • Right to Choose a Representative — A legal guardian or personal representative may exercise your rights on your behalf.
  • Right to Terminate Services — You may end services at any time without legal obligation beyond services already provided. Please notify us in session or by phone.
  • Right to Release with Written Consent — With your written authorization, any part of your record may be released to any person or agency you designate.
  • Right to Ethical Treatment — You are entitled to treatment free from discrimination based on race, ethnicity, gender identity, sexual orientation, religion, disability, age, or any other protected category.
  • Right to a Copy of This Notice — Available at the front desk or downloadable above at any time.
  • Right to File a Complaint — If you believe your privacy rights have been violated, you may file a complaint without fear of retaliation (see below).

Our Duties

We are required by law to maintain the privacy of your Protected Health Information (PHI) and to notify you following a breach of unsecured PHI. We reserve the right to change the privacy practices described in this notice. If we revise our policies, we will provide you with an updated Notice. Unless you are notified of changes, we are required to abide by the terms currently in effect.
