Self-Pay Rates
- ✓ Initial Evaluation: $275
- ✓ Follow-Up Appointment: $175
We accept all major credit cards, HSA, and FSA payments. Payment is due at the time of service.
Good Faith Estimate
Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges before your appointment. Please contact us if you would like a Good Faith Estimate prior to scheduling.
Cancellation & No-Show Policy
We understand that unexpected circumstances arise. To ensure all patients receive timely care, we require at least 24 hours notice for cancellations or reschedules. For Monday appointments, cancellations must be made by the same hour the preceding Friday.
- New Patient late cancellation / no-show fee: $100
- Existing Patient late cancellation / no-show fee: $75
These fees are not covered by insurance and will be charged to the card on file. Repeated cancellations may result in being placed on a waiting list or discharge from the practice.
