- Right to Amend — Request corrections to information you believe is inaccurate or incomplete. Submit in writing with your reasons. We will respond within 60 days.
- Right to Request Restrictions — Ask us to limit how we use or share your health information. We are not required to agree to all requests, except where required by law.
- Right to Confidential Communications — Request that we contact you at an alternative address or by alternative means.
- Right to an Accounting — Receive a list of certain disclosures we have made outside of treatment, payment, and operations.
- Right to Choose a Representative — A legal guardian or personal representative may exercise your rights on your behalf.
- Right to Terminate Services — You may end services at any time without legal obligation beyond services already provided. Please notify us in session or by phone.
- Right to Release with Written Consent — With your written authorization, any part of your record may be released to any person or agency you designate.
- Right to Ethical Treatment — You are entitled to treatment free from discrimination based on race, ethnicity, gender identity, sexual orientation, religion, disability, age, or any other protected category.
- Right to a Copy of This Notice — Available at the front desk or downloadable above at any time.
- Right to File a Complaint — If you believe your privacy rights have been violated, you may file a complaint without fear of retaliation (see below).
